This service is provided to facilitate independence and promote integration into the community for children and adults residing in their own homes or with family members. Supports provided will include assisting and supporting an individual as they pursue independence throughout their daily life across a wide variety of platforms. Staff will provide assistance and guidance as needed to aid the individual in their activities of daily living, personal hygiene, routine household care and maintenance, shopping and money management/budgeting, medication management, socialization and relationship building, the pursuit of leisure choices, participation in community activities, or therapeutic goals. Support is provided on a one-to-one basis to maximize personal growth and freedom of choice.
This is a one-on-one service provided to adults on the SCL waiver who do not live in a staffed residence. Personal Assistance services occur in a participant’s home or in the community, and services enable participants to accomplish tasks that the participant normally would do for themselves if they did not have a disability. Personal Assistance staff members provide transportation to and from community services, activities, and appointments of the participants’ choosing. This service provides participants with hands-on assistance (as needed) to perform specific tasks, as well as providing them with the level of guidance and training required to enable them to complete various activities of daily living. Support staff may assist participants in managing their medical care, including making medical appointments and accompanying them to those appointments.
